Non-emergency transportation assistance is available for qualified patients with a medical or financial need to and from doctor or specialist visits, and other medical-related facilities. Portable Electronic Medical Records (EMR)
Each WellMed patient has an EMR designed to provide your doctor and medical team access to your medical records 24 hours a day, 7 days a week.
As a WellMed patient enrolled in specific health plans, you will receive a complimentary, secured portable device in the form of a CARE band bracelet or key chain. Each CARE Band or Key Chain contains a USB port for computer access to an individual medical summary via secured web-based pages. The summary will include general health history, PCP contact information, prescribed medications and more.

Award Winning Disease Management
As a WellMed patient, or a patient of one of our independent physicians, you are eligible to participate in our award-winning Disease Management programs should you be diagnosed with: Diabetes, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD)/Asthma or Ischemic Heart Disease (IHD).

Medication Voucher Program through Disease Management
Any patient participating in the Disease Management program is eligible to participate in a medication assistance program. Over 80 disease-specific medications are available. Medications can be picked up through select Pharmacy Partners and can be mailed to your home. Please ask your physician how you can participate.

Health Coaches
Patients participating in the Disease Management program will work with on-site Health Coaches to create a management health plan. Together, they will have regular meetings and follow up care to ensure medications are being taken and the plan is being followed.
