WellMed Residential Services (WRS) is a caring, experienced healthcare program for Medicare beneficiaries living in residential long-term care facilities. Our healthcare teams put patients at the center of caregiving with the time and attention they deserve. WRS healthcare providers include WellMed primary care physicians and nurse practitioners who see patients on a regular rotation basis at the facility.:

Services include:

• Proactive medical visits by on-site certified Nurse Practitioners
• Regular visits by primary care physicians
• Communication updates to family members regarding their care
• Complimentary Transportation Assistance to specialist appointments if medically or financially needed
• Electronic Medical Records and a medical CARE band, a portable electronic medical record.
