WellMed’s Case Managers work directly with Primary Care Physicians and Hospitalists to handle the daily care of patients in the hospital. They help arrange follow up care, home equipment and medications if needed. Case Managers also work with family members to provide patient updates and instructions for care.
WellMed’s Chronic Care-Disease Management works with patients to create a health plan specifically to meet their needs.
