Disease Management - Ischemic Heart Disease

Overview of IHD Program:

• One-on-one half hour patient assessment by an LVN Care Coordinator at a WellMed Clinic for
• Lab work for complete advance lipid profile;
• Weight and Body Mass Index,
• Risk Factor Identification,
• Assessment of Vital Signs;
• Complete medication inventory;
• Clinic sessions are for physical assessment and educational information only, no invasive procedures are performed;
• The IHD Case Management Program is a FREE program offered to all qualified WellMed/Secure Horizon members.

IHD Patient Education Topics Include:

• Monitoring of cholesterol levels such acceptable HDL and LDL levels;
• Medications and medication compliance, such as:
• Cholesterol lowering medications (as needed)
• Aspirin therapy (as needed)
• Anti-hypertensive medications (as needed)
• Low Fat / Low Carbohydrate Diet
• Benefits of physical exercise as recommended by their PCP;
• Lifestyle modifications such as smoking cessation, weight loss and controlling other risk factors.
