Disease Management - Congestive Heart Failure
Overview of CHF Program:
- One-on-one hour long clinic CHF assessment and education by an RN CHF Case Manager for the patient and a family member if needed
- Clinic and home visits are for physical assessment, medication inventory and educational information only, no invasive procedures are performed
- Follow-up telephone calls are made by an LVN CHF Care Coordinator to assess the patient's condition and to provide weekly and monthly education
- Facilitate the needs of the patient with their PCP
- Each patient is given a digital scale for recording their daily weights
- The CHF Case Management Program is a FREE program offered to all qualified WellMed/Secure Horizon members
Congestive Heart Failure Program Goals for the Patient include:
- Preventing worsening of CHF symptoms
- Maintaining current levels of physical activity and functional status
- Minimizing hospital admissions and emergency room visits
- Optimizing quality of life
CHF Patient Education Topics Include:
- Teaching the patient to monitor their condition for worsening signs and symptoms and reporting earlier
- Medications and medication compliance
- Reducing sodium in diets
- Recording daily weights and reduce/monitor fluid intake if needed
- Benefits of physical exercise as recommended by their PCP
- Importance of routine follow-up PCP appointments
- Lifestyle modifications such as smoking cessation, weight loss and controlling other risk factors
