Disease Management - Overview

The WellMed Disease Management Department works directly with its primary care physicians to assist them in monitoring and educating identified patients with Congestive Heart Failure, Ischemic Heart Disease and Diabetes. Systems are in place to track the outcomes of these patients. The physicians receive monthly reports pertaining to the outcomes of their patients.
