The Official Medical Home of the City of San Antonio

In late 2009, WellMed Medical Management became the official “Medical Home” for the City of San Antonio Department of Community Initiatives. WellMed provides health screenings, health education, and limited dental screenings daily at the City’s senior one-stop centers:

WellMed provides the same services on a quarterly basis at the remaining 74 senior centers in the City’s system. Our goal is to ensure that all San Antonio seniors have access to a primary care physician.
