Our programs are designed around maximizing functions and proactive prevention. Each WellMed patient will have an Electronic Medical Record (EMR) designed around their health care needs and medical history. Physicians and clinic staff will update records after each visit and maintain notes from specialist referrals. Our physicians have access to the EMR 24 hours a day/7 days a week.

1 Electronic Medical Records:
WellMed creates an Electronic Medical Record (EMR) for each patient containing medical information and history, notes regarding Disease Management Program participation, prescription history and more.

2 Disease Management Program History
Any patient participating in a Disease Management program will have a separate record of their program history. These records include important information such as general medical history, visit history, lab results, and health goals.
3 Medication History
The Medication History records the various prescription and medication programs a doctor has prescribed for the treatment of each patient.

4 Portable and Personal EMR
Our goal is to offer you the peace of mind knowing your medical records will be easily accessible in case of an emergency or if other medical measures are necessary. No other medical group in the area offers this type of advanced technology. WellMed will provide qualified patients their own WellMed Medical summary band or key chain. The band or key chain will be able to link to a medical summary by way of web page.

5 Member Resource Gateway
