Disease Management/HealthRight Nurse

This program works as a referral system. The HealthRight nurse is a registered RN specializing in coordinating health care through the Disease Management programs available. Qualified patients may be referred to the appropriate Disease Management programs. All programs include:

• Nutritional guidelines to help manage the disease
• Programs to help prevent the disease from progressing
• Assistance for on-going care
• SilverSneakers® Exercise Programs
• Medicine Voucher Program (MVP) - Co-payments on Disease Management prescription drugs are waived if purchased through our participating pharmacies.
• Health Coaches partner with patients to create a manageable health plan and set achievable health goals.
