The WellMed Disease Management Department is a referral program open to all eligible members who live with one or more covered chronic illnesses. Our Disease Management Nurses (Health Coaches), work directly with primary care physicians in a team approach to identify chronically ill patients to closely monitor, educate and mentor them about effectively living with their diseases such as:

• Diabetes
• Congestive Heart Failure
• Ischemic Heart Disease
• Chronic Obstructive Pulmonary Disease/Asthma

Our Medical Home Disease Management teams support patients in their effort to improve their general well-being through the use of the Chronic Care Model during planned scheduled wellness visits. During these visits patients will have the opportunity to spend time with a primary care physician to discuss their chronic illness, medications and treatment options available.

Our Primary Care Physicians and Health Coaches work very hard to stay aware of the best most current methods with which to assist patients in the management of their life with a chronic illness.

Health Coaches

Following the primary care physician meeting, patients will be afforded the opportunity to spend time with a Health Coach, a highly trained professional whose responsibility it is to coach them and give them the tools with which they may better manage their life and their disease.

A Health Coach will work directly with patients on those things that are important to them. They provide coaching for self-management support, focused education for their specific disease needs and identify additional resources to assist in their disease management. Resources like medication assistance, access to social workers when needed, provision of diabetic testing supplies and even diabetic shoes are just some of the resources at their disposal.

We also offer group classes and one-on-one sessions on diabetes and heart disease through our Certified Diabetes Educators and highly trained nurses with significant experience with treating patients with these chronic illnesses.
