When you are admitted as a patient into a hospital, we assign a personal Case Manager to help you through every step of your health care process. Your Case Manager will help coordinate your WellMed Primary Care Physician, Specialists and every stage of your hospitalization, all the way through your discharge. Your Case Manager also will help you with any necessary paperwork and SNF. No matter how big or small, your Case Manager will be there to help you through any situation.
